The CCAA is requiring all officials receive the COVID-19 vaccination prior to working a contest hosted by the CCAA or a member institution. Please complete the CCAA Officials Vaccination Information form by selecting your sport below.

The information you provide will be secure and only be shared with the CCAA conference office staff and the sport assignor.

If you are fully vaccinated, please be prepared to provide a copy of your COVID-19 Vaccination Record Card from the CDC. You will need to upload the card as an image or PDF file.
